Dr. Moosavi and his fellow professionals at ULTIMATE Plastic & Reconstructive Surgery are honored to be able to offer the latest and most effective techniques for helping their patients look and feel incredible.
To do so while ensuring safety, maintaining the highest standards of quality and providing an excellent patient experience, the practice must have financial policies in place.

Reservation Fee

A reservation fee of $500.00 is required at the time of booking your procedure. This fee is deducted from the total cost of the procedure.


Payment for any surgical procedures is due in full 2 weeks prior to surgery date. Dr. Moosavi accepts payment by cash, check, or credit card.  If payment is not received by required date, the surgery will be cancelled by the office and all applicable cancellation fees will apply.

Surgical center and hospital fees and anesthesia are paid separately by telephone or in person. Each surgical facility and has specific payment policies separate from the payment policies of Dr. Moosavi.


If you are in need of financing your surgical procedure, you may apply for assistance through Alphaeon Credit or Care Credit. There may be additional fees that apply with using these services. See below for a link to their websites. Click “Apply” button for further instructions. Please note, Alphaeon and Care Credit assistance may be available for Surgeon’s fee only. 

Surgery Rescheduling

All surgical procedures rescheduled with less than 3 weeks’ notice will be subject to a scheduling fee of $100.00.

Dr. Moosavi is not responsible for refunding any surgical fees, or for rescheduling fees that result from a patient’s non-compliance. The failure to follow pre-surgical instructions includes: nicotine use, failure to avoid or to take specific medications as instructed, and failure to follow day of surgery instructions.


Dr. Moosavi requires 3 weeks (21 days) notice for the cancellation of any surgery due to non-medical reasons. If the office does not receive notice of cancellation within the above stated timeframe, the $500.00 booking fee will not be refunded. If cancelling please call our office Mon-Fri to speak with the scheduling coordinator.

Before a new surgical date can be confirmed, all fees must be paid.
